3. Assessment Requests

Submitting an assessment request through the Veridia website does not, by itself, constitute acceptance of a request for services or guarantee that an assessment will be completed.

All assessment requests are subject to review by Veridia and the availability and suitability of an appropriate physician or other qualified professional.

Veridia may contact the requesting party to obtain additional information, clarify the nature of the requested assessment, confirm availability, discuss applicable fees, and establish the requirements for the assessment.

An assessment or consultation is considered confirmed only after Veridia has communicated acceptance of the request and the applicable arrangements have been established.

4. Independent Medical Assessments

Veridia’s independent medical assessments are conducted for the purposes identified in the applicable referral or assessment request.

An independent medical assessment is not intended to replace ongoing medical care or treatment. Unless expressly stated otherwise, Veridia and the professionals conducting assessments do not assume responsibility for the individual’s ongoing medical treatment or primary healthcare.

The opinions and conclusions contained in an assessment are those of the qualified professional who conducted the assessment and are based on the information available to them at the time of the assessment.

5. No Doctor-Patient Relationship Through the Website

Use of this website, submission of an online form, communication with Veridia through the website, or submission of medical records does not, by itself, establish a physician-patient relationship between you and Veridia or any physician associated with Veridia.

The website should not be used to obtain emergency medical assistance or urgent medical care. If you require immediate medical attention, contact emergency services or an appropriate healthcare provider.

6. Medical and Personal Information

Veridia may collect personal information and health information in connection with assessment requests and the provision of its services.

Because medical information is highly sensitive, Veridia takes reasonable measures to protect information in its custody or control and to manage such information in accordance with applicable privacy legislation and professional obligations.
